This Coaching Program Agreement between is between Rev.Dr. Kimmmie / Dr. Kimberly Van Duijkeren and for the purpose of coaching services.
Program Overview:
The Coach agrees to provide coaching and support to assist the Client in achieving their personal and/or professional goals through a coaching program. The coaching program will consist of a set number of coaching sessions, as agreed upon by the Coach and Client.
Coach’s Role:
The Coach agrees to serve as a coach to the Client, providing coaching and support to assist the Client in achieving their personal and/or professional goals. Coaching is a partnership between the Coach and the Client, and the Client is solely responsible for their own decisions and actions.
Client’s Responsibilities:
The Client is responsible for their own well-being and for making their own decisions. The Client acknowledges that coaching is not a substitute for professional advice by a qualified professional and is not a substitute for medical or mental health treatment.
Confidentiality:
The Coach agrees to keep all information discussed during coaching sessions confidential, except in cases where disclosure is required by law. The Client also agrees to keep all information discussed during coaching sessions confidential.
Limitation of Liability:
The Client understands that coaching is not a guarantee of any specific outcome or result. The Coach will not be held responsible or liable for any actions or inactions taken by the Client or any consequences thereof. The Client acknowledges that they have been informed that coaching is not a substitute for professional advice and releases the Coach from any liability, claims, or demands arising from or related to the coaching relationship.
Termination:
Either party may terminate this Agreement at any time, for any reason, with written notice.
By signing below, the Client acknowledges that they have read and understand this Agreement and agree to the terms and conditions outlined above.
